- Bi-weekly coordination check-ins to review priorities, follow up on the plan, and address what is most pressing for your family.
- Behind the scenes coordination including scheduling, insurance navigation, provider follow up, home care research, and benefits enrollment handled on your behalf.
- Monthly written care summary so you always know where things stand and what is coming next.
- Direct access to your dedicated care coordinator between sessions by phone, text, or email.
